Immunizations required for incoming 8th, 12th grade students

Attention 8th & 12th grade Parents/Guardians:
State law and health regulations require students to be properly immunized and provide verification to the school nurse, unless they have an appropriate exemption card from Pettis County Health Center on file. If your child is going into the 8th grade they must have one dose of Tdap and one dose of MCV (Meningococcal Conjugate). The first dose of MCV must have been administered after 10 years of age. 
Students who will be 12th graders are required to have two doses of MCV unless the first dose was administered at 16 years or older, in which case only one is required. At least one dose must be given after 16 years of age. Students will be excluded from school starting Sept. 14, 2020, if immunizations aren’t up-to-date.
The state has required these immunizations whether students are attending school in person or virtually.
Parents are encouraged to have their doctor's office or health agency fax or email the records to the appropriate office (nurse contacts are listed below) or they can scan and email the record themselves.
